Course Evaluation Forms

Download and fill out a course evaluation.

The Course Evaluation/Narrative Form is designed to give students an opportunity to share anonymously their experience of the courses at Pacific Oaks and they way they are taught.  Near the end of each semester the instructor and students are asked to take some time to fill this form out, collect it, and turn into the academic department office on the last day of class.  We expect this information to be useful in continually improving education at Pacific Oaks.
